Lakewood Football Club Participation Agreement, Release, and Medical Authorization
Please read this in full before registering.
I am the parent or legal guardian of the child named in this registration ("my child"), and I have the legal authority to sign this agreement on their behalf.
1. What my child is signing up for
My child will take part in soccer activities operated by Friends of Lakewood Elementary School ("Friends of Lakewood"), a Texas nonprofit corporation, through its Lakewood Football Club program ("LFC"). These activities include training sessions and, for some age groups, games. They take place at Lakewood Elementary School and at any other location Friends of Lakewood designates.
This agreement applies to the program season for which my child is registered. I understand I will be asked to agree to it again each time I register my child.
2. I understand this is a drop-off program
I understand that LFC is a drop-off program. Friends of Lakewood provides supervision only during the scheduled session, and only on the field or designated activity area.
I am responsible for my child before and after the session, including getting them to and from the field, and I will make sure my child is picked up promptly at the scheduled end time. Friends of Lakewood does not provide supervision before the session begins or after it ends.
3. I understand the risks
I understand that soccer is a physical, fast-moving sport and that participation carries risks that cannot be eliminated no matter how carefully the program is run. These risks include, without limitation:
collisions with other players, coaches, equipment, goals, or fixed objects
falls, trips, and impacts on uneven, wet, hard, or poorly lit ground
being struck by a ball
sprains, strains, fractures, dental injuries, concussion and other head injuries, and in rare cases catastrophic injury, paralysis, or death
heat illness, dehydration, cold exposure, lightning, and other weather conditions
reduced visibility during sessions held at or after dusk
exposure to communicable illness
the acts and omissions of other participants, parents, spectators, coaches, and volunteers
I understand this list is not complete and that there are other risks, both known and unknown.
4. I accept those risks
Knowing these risks, I voluntarily choose to let my child participate. I accept and assume all risks of injury, illness, death, or property loss arising from my child's participation, whether or not those risks are listed above.
5. Release of my own claims
PLEASE READ THIS PARAGRAPH CAREFULLY. IT AFFECTS YOUR LEGAL RIGHTS.
ON MY OWN BEHALF, I RELEASE, WAIVE, AND DISCHARGE FRIENDS OF LAKEWOOD ELEMENTARY SCHOOL, ITS LAKEWOOD FOOTBALL CLUB PROGRAM, AND THEIR OFFICERS, DIRECTORS, BOARD MEMBERS, COACHES, EMPLOYEES, VOLUNTEERS, AND AGENTS (TOGETHER, THE "RELEASED PARTIES") FROM ANY AND ALL CLAIMS, DEMANDS, AND CAUSES OF ACTION OF ANY KIND THAT I MAY HAVE ARISING OUT OF MY CHILD'S PARTICIPATION, INCLUDING CLAIMS CAUSED IN WHOLE OR IN PART BY THE ORDINARY NEGLIGENCE OF ANY OF THE RELEASED PARTIES.
THIS RELEASE DOES NOT APPLY TO GROSS NEGLIGENCE, RECKLESSNESS, OR INTENTIONAL MISCONDUCT.
I understand that this paragraph releases claims I might otherwise bring in my own name, including claims for medical expenses I pay on my child's behalf and for loss of my child's services or companionship.
6. Medical authorization
If my child is injured or becomes ill during an LFC activity and I cannot be reached, I authorize Friends of Lakewood and its coaches and volunteers to seek and consent to emergency medical care, including transport by ambulance and treatment by qualified medical personnel. I am responsible for the cost of that care.
I confirm that the medical information I have provided at registration, including allergies, medications, and conditions, is accurate and complete, and I will notify Friends of Lakewood promptly if it changes.
7. Fitness to participate
I confirm that my child is in good enough health to take part in organized soccer, and that I am not aware of any medical condition that would make participation unsafe. If a condition develops during the season, I will tell Friends of Lakewood before my child participates again.
8. Photographs and video
I understand that Friends of Lakewood photographs and films its activities, and that participation is subject to the Photo and Video Release at https://www.foldallas.org/release. I understand I may opt my child out by telling a coach or director on arrival, or by emailing Friends of Lakewood.
9. Program changes and cancellations
I understand that refund requests are handled individually, and that I can contact Lakewood Football Club with questions. Contact info can be found on https://www.foldallas.org/soccer.
10. Behavior
I agree that my child, and I as a spectator, will follow the directions of coaches and staff and behave respectfully toward players, coaches, officials, and families. I understand Friends of Lakewood may remove a participant or spectator from an activity for conduct that is unsafe or seriously disruptive.
11. General
This agreement is governed by the laws of the State of Texas, and any dispute will be brought in the state or federal courts serving Dallas County, Texas. If any part of this agreement is found unenforceable, the rest stays in effect. This agreement binds me, my child, and our heirs and representatives.
12. Electronic signature
By checking the box at registration and submitting payment, I am signing this agreement electronically. I intend that checkbox to have the same legal effect as my handwritten signature, and I confirm I have read and understood this agreement, that I have had the chance to ask questions, and that I am signing it freely.
